Phytel, the leader in automated physician-led population health improvement, announced today that Northeast Georgia Physicians Group (NGPG), part of Northeast Georgia Health System and one of the largest multispecialty groups in Northeast Georgia, has selected Phytel and its Atmosphere™ platform to help transform all of its 30-plus ambulatory-care clinics into patient-centered medical homes by the end of this year.
NGPG, which has been using Phytel's registry and Phytel Outreach™ solution for some time, will add the Phytel Insight™ and Phytel Coordinate™ applications this summer. Phytel Insight enables healthcare organizations to identify patient care gaps and to measure their performance in population health management, and Phytel Coordinate delivers patient interventions in support of care management. The National Committee for Quality Assurance (NCQA) allows physician practices that use these and other Phytel solutions to automatically meet certain NCQA requirements for recognition as patient-centered medical homes.
NGPG realized that, in order to scale up the requisite functions quickly, it would need a proven solution that could be interfaced with its ambulatory-care electronic health record. The group was looking for a best-of-breed population health management solution, and with Phytel's domain expertise it was able to meet all its requirements.
"We chose Phytel because their product best matched our patient care needs," said Paul VerValin, CMPE, MBA, MS, president and chief administrative officer of NGPG. "Phytel not only has the tools we need to help transform our physician practices into patient-centered medical homes, but it also provides us with guidance on how to use them. We anticipate Phytel will help automate and scale our care management process to improve efficiency and enhance care."
Steve Schelhammer, CEO of Phytel, commented, "Northeast Georgia Physicians Group selected Phytel because its management understands that automation is the key to effective population health management. We are pleased to work with such a well-regarded institution that shares our strong values and commitment to continuous quality improvement."
The premier company empowering physician-led population health improvement, Phytel provides physicians with proven technology to deliver timely, coordinated care to their patients. Phytel's state-of-the-art registry, which now encompasses more than 20 million patients nationwide, uses evidence-based chronic and preventive care protocols to identify and notify patients due for service, while tracking compliance and measuring quality and financial results.
